カナダ・アルバータ州政府は13日、7月1日から地域薬局でのリフィル処方に対し、20カナダドル（1,560円）の処方料を支払うと発表しています。

リフィルというと前回の処方を患者さんの状態を確認した上で、同じ処方のものを調剤するというのが一般的な認識ですが、アルバータ州の場合は2007年から、一定の条件の下で、量の調節や処方の変更も可能となっています。（麻薬を除く。TOPICS 2006.06.02 旧サイト）

上記によれば、下記の条件を満たして登録簿に載っている薬剤師にのみこの権限が与えられているようです。
- 少なくとも2年以上常勤で患者ケアを直接行っていること
- 過去5年間に処方に関する教育とトレーニングを行っていること
- 2人以上の医療専門職（うち一人は処方医）との共同関係（collaborative relationships）が確立していること
- 知識・技術・態度などの必要とされる能力と臨床判断力を有すると共に、薬物療法について安全かつ効果的にマネジメントが行えるだけの臨床判断が可能となるための情報などへのアクセスが可能であること
アルバータ薬剤師会（The Alberta College of Pharmacists (ACP)）では、今回の業務は業務基準（Standards of Practice for Pharmacists and Pharmacy Technicians）の範囲内で行われるとして、下記の4項目について確認した上でリフィルが可能であるかどうか判断されるとしています。
- Is the patient’s treatment current?
現在行われている薬物治療に関するものか？ - Do they have enough information about the patient’s condition?
患者さんの健康状態に関する十分な情報があるか？ - Are they familiar with the patient and their health history?
患者と患者さんの健康歴をよく知っているか？ - In their professional judgment, is refilling the prescription in the patient’s best interest?
専門的判断において、リフィルを行うことが患者さんにとって重要となっているか

各紙によれば、アルバータ州では医師による新たな処方だと35ドルかかるので、これを地域薬剤師にやってもらう（20ドルですむ）ことで年間2000万ドルの節減につながるとしています。
一方で、アルバータ州政府はジェネリック医薬品の償還の引下げを行い、8300万ドルの節減を行う他、過疎地の薬局に対し補助を行うことも明らかにしています。
アルバータ州には約4200人の薬剤師がいるそうですが、調剤はテクニシャン、患者管理は薬剤師といった役割分担が確立していること、昨年83000人に対しインフルエンザの予防接種を行うなどの実績も、こういった業務を可能にしているのでしょう。（日本ではいつのことになるのでしょう）
