慢性疾患対策に期待される薬剤師の職能（米CDC）

　TOPICS 2012.09.08 で、CDC（米疾病対策センター）が地域薬剤師を活用した高血圧対策教育プログラムを開始したことを紹介しましたが、記事を書いていてどうしてこんな取り組みができるのかと不思議に思っていたのですが、この少し前にCDCでは下記のような Guide も出していました。

　英語力が乏しいので紹介しきれない部分がありますが、Guide では、薬物治療管理（MTM：medication therapy management）、包括的な服薬管理（comprehensive medication management）、共同薬物治療管理（CDTM：Collaborative Drug Therapy Management）などの定義を解説した他、これらを活用して地域薬局や薬剤師を含めたチームで慢性疾患（Chronic Disease）対策にどのように取り組むかなどが記されています。

　 Guide ではチームケア（Team-Based Care）における薬剤師の役割を次のように記しています。

• The role of pharmacists in providing patient care services is compatible and synergistic
  with the patient-centered medical home model and other innovative models of
  team-based care.
• Pharmacists extend the health care team to the local community, providing patients with the resources and care they need. In addition, pharmacists are some of the most accessible health care professionals and have a broader knowledge of medicines (prescription and over-the-counter) than any other member of the health care team.(OTCの知識もないとダメということらしい）

　Guide ではCDTMの事例や、進行中の3つの事例（プログラム）を紹介しています。

 　こう見ると、Million Herats”（http://millionhearts.hhs.gov/index.html）の取組もそうですが、米国では患者のために医師・薬剤師、そして行政が Public Health のために協力して、それぞれ何ができるかを考えているんですね。

　米国の薬剤師たちの実績の賜物（エビデンスづくり）ともいえますが、これを受けて、国家が薬剤師の職能を常にどう活用しようとしているのかが改めて感じられました。

　日本の厚労省も6年制にした薬剤師を、社会でその職能をどう発揮させるかを是非検討してもらいたいものです。（何かCDTM導入に向けた試行事業を行うといいと思う）
